How Do I Sign Up?

Click on the link below under "Sign Up For My Immunizations." 


·  Enter your demographic information, including insurance information and answer all questions. 

·  Please print a copy, sign and date, and bring it with you on the day of your clinic.

·  Don't forget your mask!! 


Sign Up For My Immunizations HERE:

PLEASE DO NOT ATTEND THIS EVENT IF ANY OF THE FOLLOWING APPLIES TO YOU: 1. You have NOT been fever free (less than 100.4 F) without a fever reducer in the past 72 hours. 2. In the past 14 days have you had any cold or flu-like symptoms; Fever or chills, Cough, Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat, Congestion or runny nose, Nausea or vomiting, Diarrhea

3. You or anyone you are in close contact traveled outside of the U.S. in the past 14 days. 4. You have been in close contact with anyone who has been diagnosed with COVID-19 or you have been diagnosed w/ COVID-19 in the last 14 days

Who Should Be Vaccinated?

How Often?

Flu (Pediatric)


All children.  6 mos-35 mos



36 months and older


Hepatitis A (Adult)

-All adults who would like to be protected from Hepatitis A

2 doses provide lifetime protection

Hepatitis B (Adult)

-All adults 60 years and younger with diabetes -CPR trained staff

-Anyone whose job may expose them to human blood or other body fluids

-Anyone else who wants to be protected from Hepatitis B

3 doses provide lifetime protection  


-All adults 65 years and older

-Anyone 2-64 years who is a smoker, or who has any of the following conditions: asthmaCOPD

diabetesheart disease, or any condition that lowers the body’s resistance to infection.

1 dose at 65 years or older


1 dose before 65 years if any of the risk factors and conditions apply


-Any adult without documentation of 2 doses of MMR

2 doses provide lifetime protection


50 years and older for most insurance providers

2 doses (2-6 months apart) provides lifetime protection


Anyone 11 years and older

Recommendations vary, please ask nurse at your clinic.
